Change of Beneficiary, Name, or Assignment
The following links are for life and disability policyholders with the AICPA Insurance Trust or the AICPA Group Insurance Trust.
- Beneficiar(ies): If you wish to update your Beneficiary, please do so by logging in the AICPA Insurance Trust Online Self Service Center at www.trustmyaccount.cpai.com and submitting online. First time users must register. If you are an employee of a firm, please download, print and complete the Change of Beneficiary form. Forms may be remitted to the AICPA Insurance Trust via fax at 1-800-242-7248 or mailed to: AICPA Insurance Trust, Aon Insurance Services, 159 E. County Line Rd., Hatboro, Pa 19040.
- Change of Name: If you have a name change, complete an on-line Change of Name Form or download and print a blank Change of Name Form as a PDF file. Note: When completing this form, you will need to have the Name, Social Security Number and Account Number of the Insured.
- Assignments: If you wish to change ownership and assign your account, please complete an Assignment Form or download and print a blank Assignment Form as a PDF file. Note: When completing this form, you will need to have the Name, Social Security Number and Account Number of the Insured.
