Choose your coverage amount

Get the coverage amount that's right for you—up to $12,000 a month. Benefits begin after you have been disabled for 30 continuous days. If you're under age 65, your benefits can continue for as long as 24 months for each period of disability. For those age 65 or older, your maximum benefit duration is 12 months. In order to qualify for benefits you must be responsible for 20% or more of the firm's expenses.

You are considered disabled if you can't perform the material and substantial duties of your occupation due to sickness or injury. You must be under the care of a licensed physician, not currently working 40 hours per week, and not have capacity to work 40 hours per week. Your coverage will end the January 1 on or that follows your 70th birthday.

Use the chart below to calculate your premium. The maximum monthly benefit amount is $12,000. Subsequent changes to your rate are based on your age as of each January 1. Rates are subject to change, but only on a class basis.
